You may FAX your orders any time to fax number (310) 632-8821 using Prestige Dental Products’ "Order Form" The following information is REQUIRED:
- Office/Business name
- Telephone number and fax number
- Billing address and shipping address
- Contact name
- Item number and description i.e. 47-105871 Disposable Prophy Brush w/Flat brush 144/box
- Unit of measure
- Quantity desired
IMPORTANT NOTE
- In order to purchase items such as needles, syringes, I.V. products, anesthetics and pharmaceuticals, we are required to obtain a copy of your DEA Registration Certificate and Physician License, prior to your first order.
- Please fax to (714) 632-8274, noting with office/business name and telephone number.
INTERNATIONAL ORDERS
- Prestige Dental Products will not be responsible for Customs Fees in the destination country, nor for shipping charges resulting from the rejection of the products by the buyer or from Customs.
- Shipping costs back and forth will apply if the item(s) get rejected or denied entry.

RETURN POLICY
- An RA number is required.
- All returned merchandise is subject to inspection, and final determination, by Prestige Dental Products.
- Any merchandise returned to us in an unacceptable condition will NOT be eligible for credit/exchange.
- Pharmaceuticals, dated items, refrigerated products, and misused/abused items are NOT eligible for return/credit/exchange.
- A minimum restocking fee of 15% will be applied for dental supply returns over 30 days from the date of purchase.
- We will replace or repair defective items based on Manufacturers' policies.
